In order for us to create a new Mentored Course registration sheet and allocate the funding, please let us know when you plan your next course so we can schedule it in. Don’t forget to fill in the separate forms for Course Box requests and for your publicity materials.
Please first schedule your course before requesting a box.
We need a minimum 2 week lead time prior to when you need the box to prepare and ship it (more if there are holidays)

The Post Course Analysis (PCA) form is on the website here. Please fill it in after your course ahead of requesting your PCA with HQ. When we have your paperwork and have done the PCA, we will pay your invoice for the course
